Before filing a complaint
We suggest that you contact the insurance company or agent to inform
them of the problem. Document your contact by noting the name of the
person you speak to, the date of the call and a brief summary of the
conversation.
If you are not satisfied with the
results, our consumer service personnel are
available to help.
Our

How to File a Complaint
You may submit your complaint to us
on-line. The link to the on-line form appears below.
You will need the Free Adobe Reader Version 7.07 (or above) for the form to
correctly function. If you need to install Adobe Reader or
upgrade your current Adobe Reader to the latest version, please use
the Adobe link below. If the complaint form does not open
after you have the latest free Adobe version, please call the
department at the phone numbers above.

The on-line complaint
form requires that certain fields be completed; if not, a reminder will
appear when you submit the form. Documentation is not required to support
your complaint, however, it is helpful to our complaint analyst in understanding your problem.

Once the department receives your complaint form:
1) Your complaint will be forwarded to the insurance company
and/or
agent for their review and response.
2) Once we receive their response, we will review it
to
ensure
compliance with Utah insurance laws.
3) We will then forward our findings to
you.
4) The complaint process usually takes three to four weeks.
