Obtain a List of Licensees
A. To obtain a customized list of insurance companies, agents, or agencies with their addresses:
- Send an email - scharlesworth@utah.gov or FAX - 801-538-3829, to the attention of Silmara Charlesworth. Include your phone and fax number along with specifics about the list you want, such as: Type of Entity: agents, agencies or companies (if a company include NAIC #) Line of Insurance: life, health, property & casualty, title or bail bond
- We will contact you if we have any questions about your request.
- A cover letter with the cost of the job and a release agreement will be emailed to you for review and signature.
- Upon receipt of the payment and the relesese agreement, the list you requested will be mailed to you.
