If you suspect insurance fraud, here’s what to do:
- Don’t ignore it.
- Share as many details as possible such as names, dates, policy numbers, incident descriptions, and any documents or photos.
- Never confront the suspected person. Let investigators handle it.
- Preserve evidence.
- If you prefer to stay anonymous, many states and companies accept confidential tips through hotlines or online forms.
You can report insurance fraud in a number of ways:
- Your insurance company: Contact your company’s fraud or claims department.
- The Utah Insurance Department: Visit our Report Insurance Fraud page to report your suspicions directly to us. Most states have a fraud bureau or consumer protection division with online reporting forms. Search for “[State] Department of Insurance fraud report” to find the resource in your state.
- National Insurance Crime Bureau (NICB): A nonprofit that partners with insurers and law enforcement to fight fraud. Reports can be made anonymously.

- Medicare or Medicaid Fraud:
- Medicare: Call 1-800-MEDICARE (1-800-633-4227) or report online at Medicare.gov.
- Medicaid: Contact Utah Medicaid, or your state’s Medicaid office or Medicaid Fraud Control Unit if you’re in another state. You may also report to the U.S. Department of Health and Human Services Office of Inspector General Hotline: 1-800-HHS-TIPS (1-800-447-8477) or online at oig.hhs.gov/fraud/report-fraud.
