Provider Applications

In addition to licensing insurers, insurance agencies, and insurance producers, the Utah Insurance Department also licenses and regulates providers of insurance-related products, or products that are similar to insurance but are not insurance.

These products include:

  • Guaranteed asset protection (GAP) waiver provider offers or administers GAP waiver contracts in connection with a loan, lease, or retail installment sale of a personal vehicle.
  • Rate service organization (RSO) makes rate, rule, and form filings on behalf of its members or subscribers and may provide other services.
  • Service contract provider offers vehicle service contracts, service contracts, vehicle protection product warranties and service contracts for consumer goods over $3,700.
  • Home protection service contract provider or home warranty provider offers service contracts for components, systems, and appliances of a home directly to a homeowner or seller.

Guaranteed asset protection (GAP) providers

To become a GAP provider you must complete the application below. All completed applications must be emailed to pcforms@utah.gov. Any applications not emailed will be rejected and returned to the sender. When mailing the fee, please attach a hard copy of the application to the check. Applications will not be processed until payment is received.

NOTE: Form filings must be filed 30 days prior to use. Form filings must be submitted at the same time as the provider application. Please see the Rate and Form Filing section for form filing instructions. Please make sure that you have reviewed Title 31, Chapter 6b, Guaranteed Asset Protection Waiver Act, for compliance.

Rate service organization (RSO)

To become an RSO, you must complete the application below. All completed applications must be emailed to pcforms@utah.gov. Any applications not emailed will be rejected and returned to the sender. When mailing the fee, please attach a hard copy of the application to the check. Applications will not be processed until payment is received.

Service contract provider

To become a service contract provider you must complete the application below. All completed applications must be emailed to pcforms@utah.gov. Any applications not emailed will be rejected and returned to the sender. When mailing the fee, please attach a hard copy of the application to the check. Applications will not be processed until payment is received.

Home protection service contract provider

To become a home warranty service contract provider, you must complete the application below. All completed applications must be emailed to pcforms@utah.gov. Any applications not emailed will be rejected and returned to the sender. When mailing the fee, please attach a hard copy of the application to the check. Applications will not be processed until payment is received.

Make sure you have reviewed Rule R590-166, Home Protection Service Contract Rule and Title 31A, Chapter 6a, Service Contracts, for compliance.

NOTE: Home warranty companies NOT backed by a reimbursement insurance policy shall annually file its annual statement within 90 days after the close of its fiscal year.
